©2013 UpToDate ® Print Email Pain diary A pain diary is a way to record your child's pain over a period of one week. At the end of each day, you or the child should write down information about the day's pain. The severity is measured on a 0-to-5 scale, with 0 being no pain and 5 being the worst pain imaginable. Triggers can include foods eaten, activities, or feelings.
