The concurrent evidence of hypocalcemia (prolonged QTc interval of 0.40 sec) and chronic hypertension (left atrial abnormality, as shown by biphasic P wave in lead V1, and voltage criteria for left ventricular hypertrophy) suggests that the hyperkalemia is due to renal failure.
