With increased abdominal pressure, the pressure on the posterior vaginal wall is equilibrated by pressure on the anterior vaginal wall (upper arrows), and there is no stress on the connective tissue supports. Distally, the pressure transmitted to the rectum is resisted by the perineal body. ARW: anterior rectal wall; PB: perineal body; PVW: posterior vaginal wall.
