SOFA — Sepsis-related Organ Failure Assessment

Running score: 0 / 24Low mortality (<10%)
Respiration (PaO₂/FiO₂)
PaO₂/FiO₂ ratio in mmHg. The 3- and 4-point bands require mechanical ventilation (invasive or NIV).
Coagulation (platelets × 10³/µL)
Platelet count in thousand/µL.
Liver (total bilirubin mg/dL)
Serum total bilirubin in mg/dL.
Cardiovascular (MAP + vasopressors)
Doses in µg/kg/min; pick the highest-tier criterion that is met. Epinephrine treated as norepinephrine.
CNS (Glasgow Coma Score)
Use the sedation-free GCS where possible.
Renal (creatinine mg/dL OR urine output)
Use urine-output rule only when more abnormal than the creatinine band.
qSOFA: Respiratory rate ≥ 22
Component of qSOFA — flags positive when ≥ 2 of the 3 qSOFA criteria are true.
qSOFA: Altered mentation
GCS < 15 or any new altered mentation. Component of qSOFA.
qSOFA: SBP ≤ 100 mmHg
Systolic blood pressure ≤ 100 mmHg. Component of qSOFA.
